Our Services
Post Discharge Providers
JORELI Health Services partners with post-discharge care providers, including home healthagencies, home-based assisted living providers, and community-based services, to support coordinated transitions of care.
Our provider network is built through a structured vetting process focused on alignment with quality expectations, responsiveness, and collaboration. Facilities may request that existing provider relationships be reviewed for inclusion in the JORELI network.
JORELI Health Services does not interfere with provider operations, billing, or clinical decision-making. We serve as a coordination layer, helping providers align with facility expectations and resident needs during post-discharge transitions.
Our goal is to foster collaborative relationships that support continuity of care, clear communication, and sustainable post-discharge partnerships.
Hospitals / Nursing Homes / Case Managers
JORELI Health Services works alongside hospitals, nursing homes, and care management teams to support safe, informed post-discharge transitions. Our role is to complement existing discharge workflows—not replace them.
We provide coordination support by maintaining access to a network of vetted post-discharge providers and helping residents and families navigate available options while preserving resident choice. Facilities may also submit existing preferred providers for vetting and inclusion in the JORELI network.
Founded and led by a Licensed Nursing Home Administrator, JORELI Health Services understands regulatory expectations, discharge pressures, and post-acute coordination challenges. We operate with sensitivity to compliance, documentation, and facility workflows.
JORELI Health Services does not assign providers, direct care decisions, or interfere with billing. Our focus is on coordination, communication, and continuity—supporting care teams while respecting existing relationships.
Residents & Families
JORELI Health Services supports residents and families during the transition from hospital or facility to home or another care setting. We understand that discharge can feel overwhelming, and our role is to provide clarity, coordination, and support—without taking away your choices.
We help residents and families understand post-discharge care options and connect them with a range of vetted post-discharge providers, including home health agencies and home-based assisted living providers. All care decisions remain with the resident and their family.
Participation with JORELI Health Services is always voluntary. With consent, we may offer optional follow-up support after discharge to help ensure continuity, answer questions, and provide guidance during the transition period. JORELI Health Services does not assign providers, make medical decisions, or interfere with care.
Our goal is to help residents and families feel informed, supported, and confident as they move forward with care.
